The Yard Sports Development asks every family to sign this before an athlete trains in the building. Read it, then sign with your full name.
I release The Yard Sports Development, its owners, and its staff from liability for injuries arising from ordinary participation in training activities, to the extent permitted by law.
I understand baseball and softball training involves inherent risks, including impact from batted and thrown balls, and I accept those risks for myself or my minor athlete.
I confirm the athlete is physically able to participate and I will disclose relevant medical conditions to facility staff before training.
PLATFORM NOTICE
This waiver is between the athlete (or parent/guardian) and the Trainer identified above. Continuum Standard provides the technology platform that facilitates this agreement. Continuum Standard is not a party to this waiver, does not provide training services, and assumes no liability for injuries, losses, or damages arising from training activities conducted by independent trainers using this platform.
Trainers using the Continuum Standard platform are independent service providers. Continuum Standard does not supervise, direct, or control trainer conduct during training sessions.
This document was signed electronically in compliance with the Electronic Signatures in Global and National Commerce Act (ESIGN Act) and the Uniform Electronic Transactions Act (UETA). Electronic signatures carry the same legal weight as handwritten signatures.
PARENT/GUARDIAN ADDENDUM FOR MINOR ATHLETES
As the parent or legal guardian of the minor athlete named in this waiver, I consent to their participation in baseball training activities as described above. I acknowledge the risks outlined in this document and accept them on behalf of my child.
I understand that in certain states, a parent or guardian cannot legally waive a minor's right to bring a claim for injuries. This waiver represents my acknowledgment of the risks involved and my informed consent to my child's participation. It does not waive any rights that cannot be legally waived on behalf of a minor in the applicable jurisdiction.
I agree to inform the Trainer of any changes in my child's health, medications, or physical condition that could affect their ability to train safely.
